Patients can collect a travel vaccination form from reception and should return the form to reception once completed.  The nurse will then contact the patient to inform them of any vaccinations they may require. Please allow sufficient time before travelling, as travel vaccinations need to be ordered in to the surgery.

There may be a charge for certain travel vaccinations.  The nurse will let you know this before you decide to proceed.
